Templates are 'forms' that can be used to collect screening or survey
(self-reported) data. These templates are stored here in this shared template
area and are accessible to authorized users. Such authorized user may include
other team members who wish to use the same screening form or may be individual
dancers who have been requested to completed a particular questionaire.
To create a template, a screening form or survey form must be created and then
saved to this shared template area.
Once a screening or survey template is copied to a users's login, that screen or
survey will be used for all subsequent uses unless for the user unless the
user's particular copy is modified. If the template is deleted or modified, it
will not affect any users' copies.
